What to do if you are injured on the Job
1. If you are in injured on the job, report it to the foreman as soon as
possible. Make sure the Foreman fills out the accident report and make sure
you get a copy of the accident report
2. Get a Doctor slip. If the company refuses to give you one, immediately
contact your business agent. You have the right to a doctor’s slip. If you
are unconscious, treated by paramedics and require additional care, you will
be transported to the closest emergency facility.
3. Use the doctor slip for emergency or treating physician of your choice.
You do not have to go to any facility recommended to you by the company. You
do not have to sign any forms for release of your medical records.
4. Once treated in the emergency room, you have the right to choose a
treating doctor. You also have the right to consult with an attorney prior
to choosing a treating doctor, if you so desire.
5. Get the full names, addresses and phone numbers of witnesses.
6. You ARE NOT required to give a statement to any company or insurance
carrier representative, especially at a medical emergency facility. If
however you give statement to the company or insurance carrier, you do not
have to agree to be tape recorded.
